Here you will find some key documents for the North Powys Wellbeing Programme.  In addition, you will also find links to our strategic plans from both Powys County Council and Powys Teaching Health Board.

Following the first phase of resident/professional engagement from June 14, 2019 to September 16, 2019, we have produced our first ‘draft’ Model of Care for north Powys. The model captures the feedback and experiences from a wide range of stakeholders, including residents of all ages, harder to reach groups, staff and children of what works well now, and what they would like to see improved in the future at home, community, region, and out of county.


The North Powys Wellbeing Engagement report has supported the development of the draft ‘Model of Care’ and supporting ‘Case for Change’ to ensure our residents voice is at the heart of a new model of care through a co-production approach.
